Suigetsu is, to put it simply, a dick. He's not necessarily a bad person, but he's not nice by anyones standards. He doesn't see the point in being nice, thinking that nice is completely overrated. He's honest to the point of being cruel sometimes, because he's extremely opinionated. He's got a bit of an ego, but not enough to be a narcissist. He's not maliciously cruel, but he has the potential to be so if he wanted to be. He's a very nonchalant person, and prefers lazying around with a bottle of water and no clothes on. Being the nudist that he is, that would make perfect sense.
While Suigetsu isn't nice, he's not mean. He can be a good friend when it really comes down to it, though he's not the best at being compassionate or caring. If he likes you, he just won't choose to pick on you. His comments are more mellow and honest when he likes you and a touch sharper when he doesn't. If he likes you, he'll defend you, though probably not to the death unless you're super close. He had some parent issues due to his mother walking out on him and his father pretending he didn't exist.
To make a long story short, Suigetsu has problems with being nice, but despite his attitude, he's not a bad guy underneath it all.
Background:
When Suigetsu was very young, his mother walked out on him and his family. He spent the rest of his life in the slums with his dad, who pretended that the young boy didn't exist. He was born with Von Willebrands disease, which makes his blood thin enough to not clot quickly. Because of this he was always seen as a weak little brat in his slums neighborhood. He tried hard to be tough and succeeded. Because of his family situation, the school insisted he go to therapy. While in therapy and getting nowhere with his mother issues, he fainted from blood loss, and his temper at the time hadn't helped. He was taken to a hospital, where he ended up being kept sick for medical testing. The testing that was done to him caused his psychosis, and when it was discovered he was shipped off to Malaise.
